As of 2023, the committee is made up of

Patrick Blake, MD, Chief Medical Officer

Jay Blake, DNP, Director of Patient Care

Sandra Carnevale, Compliance Administrator

The CRC reviews Provider and Clinical Director candidates

  1. Potential new Providers and Clinical Directors will be presented to the CRC for clinical review before further consideration by the organization as a whole

The CRC trains and privileges providers

  1. Clinical privileging for both new and existing providers will be determined by the CRC

  2. Existing providers requesting new procedures will be granted after CRC review

The CRC serves as a source of knowledge for the providers

  1. On Portrait processes as well as a resource for general clinical queries and issues
  2. CRC maintains a Notion page tracking provider questions and issues
  3. CRC also maintains a Notion Page tracking cases submitted by providers

The CRC formally approves the Policies and Procedures (P&P) we use in clinic

  1. Documents and tracks decision making for any future revisions to clinical P&P as needed

The CRC reviews clinical requests from the rest of the organization

  1. Provides Clinical approval of new services or procedures
  2. Peer review for adverse events or near misses
