Charts for all providers (RN, NP and PAs) will be audited ongoing both by the Internal Review Team (IRT) and by the provider’s Clinical Director for completeness and accuracy


     **  **Portrait RN provider chart review will be 100% for the first 30 encounters. ****

          ** **Portrait RN, NP and PA provider chart review will be 10% monthly** ** 

The Portrait EHR will queue provider-specific charts for your review, which will generate a queue in Appsmith.

Once monthly, please follow the following workflow to complete the charts received 30-60 days prior (Portrait will review notes 14 days after the visit as a quick QA check)

  1. Review the “procedure notes” and “medical charting” sections for completeness as well as adherence to Portrait protocols, procedures, and dosing recommendations

                                                               **CLEARANCE (GFE) REVIEW**
    

  1. Navigate to the “Physician Review” section of Appsmith
    1. Review current photographs for patient “visit summary” section, and go to “patient details”
      1. Note any flags/contraindications for treatment

      2. Ensure that diagnoses are appropriate based on photos

      3. Ensure that treatments indicated are appropriate based on diagnoses and patient candidacy

                                                                      **VISIT REVIEW**
        

  1. Navigate to the “Physician Review” section of Appsmith
    1. Review “before and after” photos in the chart
      1. Confirm provider is taking appropriate before and after photos
      2. Review annotated photographs for dosing and injection points
      3. Note any product selection or dosage which you find inappropriate
  2. Navigate to the EHR (optional, for more detail)
    1. Take note of what services were provided
      1. Confirm that provider updated templated procedural note language for provided services
      2. Confirm dosage was in line with standing orders and policies
        1. Compare with standing protocols for dosing
      3. Take note of follow up appointments
        1. Compare with recommended follow up scheduling
